Welcome to TAF Diagnostics Services (“TAF Diagnostics,” “we,” “our,” or “us”).
These Terms of Service (“Terms”) explain the rules for using our services, scheduling with us, and using our website.

By booking an appointment, visiting our location, requesting mobile service, or accessing our website, you agree to these Terms.

If you do not agree with these Terms, do not use our services.


1. Services We Provide

TAF Diagnostics Services provides testing, verification, and compliance-related services, which may include:

Drug and alcohol testing (DOT and non-DOT)

DNA testing (legal, paternity, immigration, family relationship)

Fingerprinting (live scan and ink card)

I-9 employment eligibility verification

Apostille and document authentication assistance

Mobile / on-site collection and testing services for individuals and employers

Corporate compliance support for hiring and onboarding

We are located in Odessa, Texas and serve the Midland/Odessa area and surrounding West Texas communities.

We reserve the right to refuse or discontinue service if a situation is unsafe, abusive, fraudulent, non-compliant, or unlawful.


2. No Medical Diagnosis / No Legal Advice

We do not provide medical diagnosis, medical treatment, medical opinions, or medical advice.

We do not provide legal advice or representation.

Any test results, documentation, or collected information is provided “as is” for compliance, identity verification, employment, administrative, legal, or personal use. You are responsible for how results are used in court, with agencies, or with employers.

If you need legal advice, consult an attorney.
If you need medical advice, consult a licensed healthcare provider.


3. Identification and Documentation

To protect the integrity and legality of results:

You may be required to present a valid government-issued photo ID.

For minors, a parent or legal guardian must be present and may be required to provide proof of guardianship.

For employment or DOT testing, you may be required to provide employer authorization, custody forms, or case numbers.

For I-9 verification, only original, acceptable documents will be accepted. We cannot accept photocopies, screenshots, or expired documents unless permitted by law.

We reserve the right to deny service if identity cannot be verified, documentation is incomplete, or we believe fraud may be occurring.


4. Chain of Custody and Compliance

For services such as drug testing, alcohol testing, and legal/immigration DNA collections:

We follow chain-of-custody and identity verification procedures.

You agree to cooperate with those procedures (providing ID, signing forms, initialing seals, etc.).

Interference, falsification, tampering, or refusal to cooperate may be documented and reported to the requesting employer, agency, attorney, court, or authority.

For DOT-regulated testing, we follow DOT Part 40 procedures.


5. Appointments, Rescheduling, and Mobile Dispatch

Scheduling

Appointments can be made online, by phone at (432) 315-2390, or by email at [email protected]. Some services allow walk-ins, but availability is not guaranteed.

Rescheduling / Cancellations

We ask that you notify us as early as possible if you need to change or cancel.

For mobile/onsite corporate testing:

Rescheduling may require advance notice.

Late cancellations may result in a call-out or dispatch fee.

Dispatch / Mobile Services

For on-site or mobile testing at your location:

A minimum service volume or mobile fee may apply.

We reserve the right to decline or delay dispatch for safety, staffing, distance, or compliance reasons.


6. Payment and Billing

Payment Terms

Payment is due at the time of service unless other arrangements are approved in writing (for example, employer billing or legal invoicing).

Some services (for example DNA testing, certain document services, and mobile dispatch) may require prepayment, a deposit, or a retainer. Deposits are generally non-refundable once collection or document processing has begun.

If a third party (attorney, employer, staffing agency, etc.) has agreed to pay and fails to do so, you may still be responsible for the balance.

Refunds

Because many of our services involve chain-of-custody, lab processing, or official document handling:

All sales are final once the sample is collected or the document handling process has begun.

We do not guarantee any specific legal, immigration, employment, or court outcome.

We do not provide refunds if:

You fail or refuse a test.

A hiring decision, legal decision, or court decision is not in your favor.

A government agency or employer rejects results for reasons outside our control.


7. Results, Reporting, and Turnaround Times

We may provide estimated turnaround times (for example, “3–5 business days” for DNA, or “24–72 hours” for lab-confirmed drug screens). These are estimates only.

Timing depends on:

Laboratory processing speed

Shipping / transit

Government/agency backlog

Completeness of ID or paperwork

We are not responsible for missed deadlines with courts, immigration, employers, schools, or background screening due to processing delays that are outside our direct control.

Some results may be delivered only to the requesting party (for example, employer or attorney), not directly to the individual tested. You acknowledge and agree to that structure where applicable.


8. Privacy and Confidentiality

We take confidentiality seriously and handle your information according to our Privacy Policy.

By using our services, you consent to:

Our collection and storage of identity information, biometric data, and/or biological samples.

Our documentation of the testing event.

Our release of results to authorized or legally required parties (for example, your employer, your attorney, the court, DOT auditors, USCIS, etc.).

9. Behavior and Safety

We provide professional, compliant, and respectful service. We expect the same in return.

We reserve the right to refuse service, end an appointment, or request that someone leave the premises if any of the following occurs:

Threats, aggression, verbal abuse, or harassment of staff or other clients

Attempted sample tampering or fraud

Filming or photographing in restricted areas

Refusal to follow safety or privacy rules

On-site/mobile: If conditions are unsafe, unsanitary, unlawful, or inappropriate for a compliant collection, our team may leave and the service will still be billable as a dispatch.


10. Website Use

When you use our website or booking tools, you agree to:

Provide accurate information

Not attempt to access or interfere with systems, forms, or records that are not yours

Not submit false identities or stolen credentials

Not attempt to reverse engineer, scrape, or misuse our booking systems

We may suspend online access or block IPs involved in abuse, fraud, harassment, or automated scraping.


11. Intellectual Property

All branding, logos, text, graphics, service descriptions, and marketing materials displayed on our website and in our printed materials are the property of TAF Diagnostics Services, unless otherwise noted.

You may not copy, resell, rebrand, or republish our materials without written permission.


12. No Guarantee of Outcome

We provide testing, collection, and verification services.
We do not guarantee:

Employment approval or hiring decisions

Immigration approvals

Custody or court outcomes

Apostille acceptance by a specific foreign authority

That an employer, agency, or court will accept a late or altered sample

Our responsibility is to collect and process in a compliant manner and provide documentation consistent with our standards.


13. Limitation of Liability

To the fullest extent allowed by law:

We are not liable for indirect, incidental, special, or consequential damages, including job loss, denial of employment, legal outcomes, immigration outcomes, loss of custody, or contract loss, even if we were advised such outcomes were possible.

Our total liability for any claim connected to our services will not exceed the amount you actually paid to us for that specific service.

Some states do not allow certain liability limits, so parts of this section may not apply to you.


14. Indemnification

You agree to indemnify and hold harmless TAF Diagnostics Services, its employees, owners, contractors, affiliates, and collection partners from any claims, damages, costs, or liabilities that arise from:

Your misuse or alteration of test results

False information you provided

Your noncompliance with employer, court, or agency rules

Your violation of these Terms


15. Changes to These Terms

We may update these Terms from time to time.
When we update them, we will change the “Effective Date” at the top of this page.

Your continued use of our services after an update means you accept the updated Terms.


16. Governing Law

These Terms are governed by the laws of the State of Texas.
Any dispute must be brought in a court located in Texas, unless prohibited by law.
